
Transition planning is a component of the
adolescent’s treatment for successful progression through the service
continuum. The Transition Worker will assist the adolescent through
transitional periods or assist with a coordination of services as he/she
is being prepared to either leave or enter the treatment centre.
Intervention is meant to be short.
Priority is given to Day Treatment and Residential
adolescents as they are leaving treatment, especially for those whose
parents have not been involved in treatment or if the adolescent’s
behaviour remains somewhat difficult.
Transitional support is given if it is felt that
he/she will need additional assistance in the community (home, school)
after leaving any Frontenac program.
